Insurance & Policies

Foothills Family Practice uses the following policies to help us provide quality care to all of our patients.  If you have questions or concerns about any of these policies, please discuss them with your provider or any of our office staff on your next visit to our offices.

Insurances Accepted

We accept most major insurance plans including Blue Cross Blue Shield of Tennessee, United Healthcare, Cigna, Humana, and Medicare among others.  We are not currently accepting new TennCare patients.  It is the patient’s responsibility to verify they have coverage for any services rendered. When you make an appointment, have your insurance information on hand to verify that we accept your coverage. Ultimately the patient is responsible for all charges incurred.  If insurance does not reimburse Foothills Family Practice for the amount billed, we will bill the patient and require payment prior to providing future services.

Prescription Refill Policy

Prescription refills should normally be handled during your routine physical with one of our providers.  If you need a refill and do not have an appointment scheduled, please contact our office during normal business hours.  We will respond to your request on the same business day with notification of your refill or a request for you to schedule an appointment.  Many refill requests will require us to see you in the office to make a proper decision about your medications.  This is for your own health and safety.  After hours call providers will not respond to prescription refill requests.

Cancellation Policy

When you schedule an appointment, we block a provider’s time for you and therefore cannot schedule other patients at that time.  We must efficiently schedule our providers to be able to provide services to you at a reasonable cost.  We kindly ask that you give us at least 24 hours notice if you need to cancel an appointment for any reason.  Should you cancel and fail to give us proper advance notice on three occasions, we reserve the right to ask you to transfer your care to another practice.

Payment Policy

Payment is due when services are rendered.  For patients who have an insurance mandated co-payment, we are required to collect this co-payment at the time of services. It is Foothills Family Practice policy to collect co-payments before services are rendered.  Any other patient balances are due in full within 30 days of the service date.  Should payment not be made within 30 days, we reserve the right to add a finance charge.  If you have questions about payment or a balance due, contact our office during regular business hours.

Customer Satisfaction Policy

Our desire is that you are able to live life with good health and happiness.  We work extremely hard to partner with patients each day to provide this experience.  However, we are not perfect.  If you have received care or had communication with our office that you feel falls short of this standard, please use the feedback form on the right side of our website.  Alternately, you may contact our office.  We want the best for you, and we want to be known as a provider that genuinely cares about its patients.  We feel this sets us apart from other healthcare options, but more importantly it is just who we are.
